Privacy Policy:

For proper treatment it is necessary for the treating therapist to create a file. This is also a legal obligation imposed by the Medical Treatment Contracts Act (WGBO). Your file contains notes about your state of health and details of the examinations and treatments performed.

The file may also include information that is necessary for your treatment and that the treating therapist, after your explicit consent, has requested from another healthcare provider, such as the general practitioner.

We do our best to protect your privacy. This means, among other things, that we:

1.          Handle your personal and medical data with care;

2.          ensure that unauthorised persons do not have access to your data.

3.          Your treating therapist is the only one who has access to the data in your file.

4.          Your treating therapist has a legal duty of confidentiality (professional secrecy).

The data from your file can also be used for the following purposes:

1.          To inform other healthcare providers, for example when the therapy has been completed

  or when a referral to another practitioner is made. This will only be done with your explicit consent.

2.          For use in observation, during my absence.

3.          For the anonymized use during peer review.

4.          A small part of the data from your file is used for the financial administration, so that the treating therapist or his administrator can draw up an invoice.

If the treating therapist wants to use your data for another reason, they will first inform you and explicitly ask for your permission.

Privacy of the healthcare invoice:
The healthcare bill you receive contains the information requested by the health insurer, so that you can claim this invoice from your health insurer.

1.          Your name, address and place of residence

2.          Your customer number and/or your date of birth

3.          Date of the treatment

4.          The type of consultation you underwent

Rights regarding your data:

You have the right to inspect, rectify or delete the personal data we have received from you. You can also object to the processing of your personal data (or part of it) by us or by one of our processors. You also have the right to have the data you provide transferred by us to yourself or directly to another party on your behalf. We may ask you to provide identification before we can comply with the aforementioned requests.

If we are indeed allowed to process your personal data on the basis of your consent, you always have the right to withdraw this consent.

Privacy Policy:

Voor een goede behandeling is het noodzakelijk dat de behandelend therapeut een dossier aanlegt. Dit is ook een wettelijke plicht opgelegd door de Wet op de geneeskundige behandelingsovereenkomst (WGBO). Uw dossier bevat aantekeningen over uw gezondheidstoestand en gegevens over de uitgevoerde onderzoeken en behandelingen.

Ook worden in het dossier gegevens opgenomen die voor uw behandeling noodzakelijk zijn en die de behandelend therapeut, na uw expliciete toestemming, heeft opgevraagd bij een andere zorgverlener bijvoorbeeld bij de huisarts.

Wij doen ons best om uw privacy te waarborgen. Dit betekent onder meer dat wij:

·         zorgvuldig omgaan met uw persoonlijke en medische gegevens;

·         ervoor zorgen dat onbevoegden geen toegang hebben tot uw gegevens.

·         Uw behandelend therapeut heeft als enige toegang tot de gegevens in uw dossier.

·         Uw behandelend therapeut heeft een wettelijke geheimhoudingsplicht (beroepsgeheim).

De gegevens uit uw dossier kunnen ook voor de volgende doelen gebruikt worden:

·         Om andere zorgverleners te informeren, bijvoorbeeld wanneer de therapie is afgerond of bij een

verwijzing naar een andere behandelaar. Dit gebeurt alleen met uw expliciete toestemming.

·         Voor het gebruik bij waarneming, tijdens mijn afwezigheid.

·         Voor het geanonimiseerde gebruik tijdens intercollegiale toetsing.

·         Een klein deel van de gegevens uit uw dossier wordt gebruikt voor de financiële administratie, zodat

de behandelend therapeut of zijn administrateur een factuur kan opstellen.

Als de behandelend therapeut om een andere reden gebruik wil maken van uw gegevens, dan zal deze u eerst informeren en expliciet uw toestemming vragen.

Privacy op de zorgnota:

Op de zorgnota die u ontvangt staan de gegevens die door de zorgverzekeraar gevraagd worden, zodat u deze nota kan declareren bij uw zorgverzekeraar.

·         Uw naam, adres en woonplaats

·         Uw klantnummer en/of uw geboortedatum

·         de datum van de behandeling

·         Type Consult.

Rechten omtrent uw gegevens:

U heeft recht op inzage, rectificatie of verwijdering van de persoonsgegeven welke wij van u ontvangen hebben. Tevens kunt u bezwaar maken tegen de verwerking van uw persoonsgegevens (of een deel hiervan) door ons of door één van onze verwerkers. Ook heeft u het recht om de door u verstrekte gegevens door ons te laten overdragen aan uzelf of in opdracht van u direct aan een andere partij. Wij kunnen u vragen om u te legitimeren voordat wij gehoor kunnen geven aan voornoemde verzoeken.

Mogen wij uw persoonsgegevens verwerken op basis van een door u gegeven toestemming hiertoe, dan heeft u altijd het recht deze toestemming in te trekken.
